Certificate of Liability Insurance

The annual Certificate of Liability Insurance form (PDF) is distributed to all Extension county and Knapp Hall offices by the Extension Service Mail Center. This covers General Liability Policy and Automobile Policy for West Virginia University for the coverage period: July 1 to June 30.

A current copy of this certificate should be placed in all State Vehicles and a copy maintained in your office. Additional copies can be made as needed.

Information on what you should do if you have an auto accident can be found on the reverse side of the certificate.
